Care Management is a team-based approach to patient care that focuses on the clinical and social needs of the patient. It encompasses care coordination, self-management support and outreach tools. As an NCQA accredited Patient-Centered Medical Home, BCH strives to take care of the whole patient, to help them achieve the health goals they have set for themselves. Our Care Management staff is instrumental in achieving this goal by providing expertise in health education, access to community resources and assistance with coordination of care strategies. This is a free service available to all BCH patients and their support systems.`
