Backpack Program
Our Care Management team offers a free backpack, filled with school supplies to all school-age children who complete an annual wellness visit at Bay Community Health. This is a free service available to all eligible patients, regardless of their insurance carrier.
School sports are also back in session, many of which require participants to have a recent sports physical by a licensed provider. Sports physicals differ from annual wellness visits and are often a separate appointment. Visit your local public school district’s website to access the school health, sports physical forms and more for your child’s appointment.
